Otohime was sedated before she got shot. That would explain why she acted strangely calm during the whole incident, and why she didn't seem affected of being shot (remember, she yelled out in pain when she broke a hand). And it is also the answer to why she didn't saw it coming with her CoO - simply because she was too groggy to use Haki.
So I guess it's some sort of conspiracy. Some guy, maybe a spy in the army, made his way into the castle earlier that day and gave Otohime a sedative without her noticing. Meanwhile they stationed some assassins on the plaza to snipe her when she was unattentive.
And the group behind that conspiracy might be the same as the mysterious "shadow" behind Fishman Island.
Bullets travel faster than pain. You can die without even realizing you were shot.
In real life, yes. But in One Piece, when people are shot, they usually cramp together in pain or die/faint immediately. We haven't seen another like Otohime who slowly died without really expressing pain.
Actually, most fatal gunshot wounds in real life would give you a minute or two to live before they killed you. Very few areas on the body can be shot for an instant kill, and those that can are extremely difficult to hit on a moving target.

OK here's a theory I'd like to get this off my chest: I think Buggy is a shishibukai now.
Why? Because Oda has become rather unimaginative with his forshadowing or let's say more straightforward (examples are Luffy's tattoo message and Sabo), so yeah I'm afraid that the bat meant he was being recruited as a shishibuki.

It could also be that the AK possessed great enough technology to carve into the Poneglyphs (though I'll admit that this is a kind of baseless and mostly an extension of my personal favorite theory of their kingdom and the world back then being something like WoT's Age of Legends. Still, if they could make weapons great enough that just one of them could bring about armageddon...).
We'll see the King of Goa Kingdom at Reverie when the Fishman Isle arc is finished.
